Looking Beyond Opioids: Exploring Safe Alternatives

Opioids can be appropriate for some patients. For others, exploring alternatives can offer safer longer-term relief and improve daily function and quality of life. When a patient’s Texas Prescription Monitoring Program (PMP) history shows they have started or have a prolonged use of one or multiple controlled substance prescriptions, it’s a good opportunity to pause and explore other strategies.

Nonopioid Medications That Support Function and Relief

Several nonopioid pharmacologic therapies can help manage subacute and chronic pain. Depending on the condition, medications such as serotonin-norepinephrine reuptake inhibitors (SNRIs), gabapentinoids or topical agents may be a better first-line or long-term option than opioids. Additionally, over-the-counter options like oral and topical nonsteroidal anti-inflammatory drugs (NSAIDs) and acetaminophen remain effective for both acute and chronic pain.

CDC guidance encourages clinicians to match treatment options to pain type and patient preference, noting that long-term opioid use is rarely sustainable.

Nonpharmacologic Therapies

Many noninvasive, nonpharmacologic approaches can improve pain and function and are worth discussing with patients. Examples include:

  • Acupuncture
  • Chiropractic care
  • Massage
  • Movement-based therapies like physical therapy, exercise or stretching
  • Psychological therapies like cognitive behavioral therapy or guided breathing

These alternatives can support recovery, especially when combined with other treatments. Even a brief mention can show patients you’re focused on supporting their overall well-being and are committed to finding effective pain relief.
